Listed above are the forms available for new patients. All of the forms need to be printed and filled out before the appointment, to help us better serve you in a timely manner. Please remember that depending on your INSURANCE, you may not be tested the same day of your appointment. We do request that you arrive 15 minutes earlier than your appointment time. Thank you.

 

ANTIHISTAMINES – Avoid for 5 days prior to appointment.

If you are unable to remain off antihistamines, please let the doctor &/or nurse know.Taking any medications such as the ones listed below will cause a false reading in the test results.

  • This includes Benadryl, or any products containing Benadryl (i.e. Tylenol PM & Motrin PM), Zyrtec, Xyzal, Allegra, Claritin, Clarinex, Alavert, Dimetapp Allergy, or any Cold & Sinus, etc.

  • Do not use Astelin, Astepro, Patanase, or Dymista nasal sprays.

  • ALL eyedrops should be stopped at least 3 days prior to first appointment.

  • Also do not use any steroids, such as Prednisone, for 5 days prior.

  • Please wear short sleeves for the skin testing.
